Art Class June 30th – Paper Mache Me

Please read this form carefully and be aware of registering your minor child/ward for participating in the program(s), you will be waiving and releasing all claims to injuries your child/ward might sustain arising out of the program(s). I recognize and acknowledge that there are certain risks of physical injury to participants in programs offered. I agree to assume the full risk if any such injuries, damages or loss, regardless of severity which my child/ward may sustain as a result of participating in any activities connected or associated with any such programs. I waive and relinquish all claims that I, my insurer, or my child/ward may have against the Park Board and its officers, agents, servants and employees from any and all claims from injuries, damages or loss which my child/ward may have or which may accrue to my child/ward on account of participation in the Park Board programs. The person executing the Release and Indemnity Agreement above hereof understand that the event may be videotaped or photographed and agree to such images and further agree that images will be the sole property of the Wahpeton Park Board and may be used for advertising and promotional use without notice to or consent of a participant, parents, or guardian and that any revenue derived therefrom shall be solely that of the Wahpeton Park Board.
